Connecting to Care – A Hotspotting Initiative

Connecting to Care is a new team of health care providers in Regina Qu’Appelle Health Region (RQHR). This video was produced to show other RQHR staff how the team has been developed, and some client stories about how the program is working. In Saskatchewan, it’s estimated that one per cent of patients account for 21 per cent of hospital costs. Connecting to Care works to identify and connect complex needs patients with alternative and more appropriate services outside hospitals – so they don’t have to repeatedly visit emergency departments to get help and don’t face preventable hospital admissions.
