Hyper PFNA Surgical Technique for Proximal Femur Fracture | Sorath Ortho Welcome to the official surgical technique demonstration by Sorath Ortho, a trusted manufacturer of high-quality orthopaedic instruments and implants. This video demonstrates the step-by-step surgical technique of the Hyper PFNA (Proximal Femoral Nail Antirotation) system for the treatment of proximal femur fractures, including unstable intertrochanteric and subtrochanteric fractures. The Hyper PFNA system is designed to provide superior rotational stability, angular fixation, and optimal load distribution, especially in osteoporotic bone. Indications The Hyper PFNA system is indicated for: • Stable and unstable intertrochanteric fractures • Subtrochanteric fractures • Reverse oblique fractures • Osteoporotic fractures • Selected pathological fractures Surgical Technique Overview 1. Patient Positioning & Fracture Reduction The patient is positioned supine on a fracture table. The affected limb is placed under traction, and closed reduction is achieved under C-arm fluoroscopic guidance. Proper alignment is confirmed in both AP and lateral views. 2. Entry Point A small incision is made proximal to the greater trochanter. The correct entry point is identified at the tip or slightly medial to the greater trochanter in the AP view and centered in the lateral view. 3. Guide Wire Insertion A guide wire is inserted into the medullary canal through the entry point, ensuring central positioning under fluoroscopic control. 4. Canal Preparation Sequential reaming is performed if necessary, depending on the canal diameter. 5. Nail Insertion The appropriately sized Hyper PFNA nail is assembled with the targeting jig and inserted over the guide wire using controlled movements until optimal depth is achieved. 6. Femoral Neck Guide Wire Placement Using the targeting device, a guide wire is inserted into the femoral neck and head. Central positioning in the lateral view and central/inferior positioning in the AP view is confirmed. Proper Tip Apex Distance (TAD) is maintained to minimize the risk of cut-out. 7. Blade Measurement & Preparation Blade length is measured using a depth gauge. The lateral cortex is opened, and reaming is performed if required. 8. Helical Blade Insertion The Hyper PFNA helical blade is inserted over the guide wire to the predetermined depth. The helical design compacts cancellous bone, providing enhanced rotational stability, especially in osteoporotic patients. 9. Distal Locking Distal locking is performed using static or dynamic mode under fluoroscopic guidance using a freehand technique if required. 10. End Cap Placement & Final Assessment An end cap is inserted, and final implant positioning is confirmed in AP and lateral views before wound closure.
