Thoracotomy in steps- we may have evolved into minimally invasive thoracoscopic surgeries over last 2 decades or more but in many situations all we need is a good thoracotomy. a simple good thoracotomy is the key to a successful outcome. steps : Incision- 5th Intercostal Space posterolateral Thoracotomy 2 fingers below tip of scapula. extended through subcutaneous fat. upto latissimus dorsi. LD incised. thereafter serratus anterior muscle incised. upto the intercostal muscle. scapula elevated. Intercostal spaces calculated. from above downward. Upper border of the 6th rib i.e the 5th IC space entered carefully and thoracotomy completed under vision. post surgery ICD placed under vision and fixed. closure: Ribs closed with no 1 Vicryl in figure of 8 Sutures. Muscle in 2 layers with no 1 vicryl. skin with absorbable or non absorbable Sutures as per Institute protocol. compression dressing given.
