Paranoia: A Chronic Neuropsychiatric Symptom of Traumatic Brain Injury.

Following Brain Injury, many patients experience increasing paranoia and hallucinations. Paranoia these patients experience is of a delusional nature as even if there is strong evidence disproving the basis for the belief, the patient cannot be convinced otherwise. There is often times the presence of auditory hallucinations...more often in the form of sounds as opposed to voices specifically. We discuss the neurobiology behind the paranoia and hallucinations and treatment as there are multiple options. These neuropsychiatric symptoms leave patients isolated and often cost family and friends because of the patient's insistence and the difficulty in convincing them that they are wrong despite evidence to the contrary. We also discuss vulnerability to Conspiracy Theory Type Thinking and its basis following TBI and how it is connected with paranoia. One of the best ways to identify this problem is to recognize the all or nothing way patients approach discussion and the Negative Attribution Bias associated with their other symptoms. Everyone but themselves is bad, or against them if they don't agree and they are the only one without fault or error. This is a sign that it is the brain injury talking and which needs treatment. In any case: Treat All Patients, Survivors, Friends, Loved Ones with respect...it isn't their fault if they have this problem. They need treatment, not judgement. They don't need excuses either.
