This is a case shared by Dr. Aureliano Moreno from Tijuana Mexico In this case, anpatient with diabetic retinopathy, was submitted to PPV due to a dense vitreous hemorrhage associated with tractional retinal detachment. It´s important in theses case performing circumferential dissection of the posterior hyaloid in the midperiphery, so you can remove the tractions that connect the periphery with posterior pole and can get better visualization. After removing the blood and posterior hyaloid, we can see a stiffened retina, with pre retinal membranaes. The surgeon was able to remove most of the tractions with the vitrector in this surgery. For segmentation it’s important to use low vacuum to avoid iatrogenic breaks. The use of Chandelier light is also important, so you can perform bimanual techniques using forceps and scissors if necessary. It’s common the presence of bleeding while removing membranes from the arcades and optic disc. Be prepared to perform hemostasis with an endocautery if necessary. After removing all the vitreous and tractions, the surgeon performed a retinotomy to aspirate all the dense subretinal fluid. The use of PFCL was unnecessary in this case and after silicone oil injection, Panretinal Photocoagulation was completed.
