Orthopedic Trauma Surgery - Dr. Kamal Aderibigbe

Dr. Kamal Aderibigbe discusses orthopedic trauma and life as an orthopedic trauma surgeon. As an orthopedic surgeon, I wear two hats – I am fellowship-trained hip replacement surgery and I'm also a fellowship-trained orthopedic trauma surgeon. What are the most common orthopedic trauma cases you see? On days when I'm not doing hip replacement surgery, I'm taking care of trauma patients with a complex multitude of injuries anywhere ranging from upper extremity injuries, shoulder, elbow, forearms – to hip fractures, pelvic fractures, hip socket fractures and fractures of the knees and lower leg. Talk a little bit about life as an orthopedic trauma surgeon and how that is different than other orthopedic specialties. One of the rewarding and interesting parts of being an orthopedic trauma surgeon is I have to be current with medical treatments and also be well-versed in taking care of all kinds of injuries. A lot of times I do not know what cases I will be doing the next morning, which means I have to be always prepared. A typical day starts around 4:30 a.m. in the morning. When I wake up I log on to the computer and look at x-rays. I take a look at injuries that come in the night before and plan those surgeries. A normal day can go anywhere from about 4:30 to 5 a.m. in the morning to 6 or 7 p.m. and sometimes into the late hours of the night. But certainly, this is also a very rewarding field because we do make an impact in terms of being able to help people who have unexpected life events get back to their norm. What are factors that increase the risk of a traumatic orthopedic injury? So, in terms of trauma, a lot of these are not planned. Falls from height, automobile accidents, slippery surfaces where people slip and fall – but certainly some of those trauma injuries can also be prevented. Certainly being able to adequately forecast what's safe and what's not safe like limiting heights. One of the injuries we see a lot of will be during the seasonal periods is when people are hanging lights for Christmas when people fall from their rooftop. So again, a lot of these injuries happen to citizens and individuals who are just going about their everyday business. And part of my charges to be at my optimum best, to be able to deliver world-class care and get this patients back to their normal position. Talk about the difference between life-threatening and non-life threatening orthopedic injuries. So in terms of our trauma patients, when they initially present to the hospital, these patients are initially seen by the general surgery trauma team. They make sure that the patient does not have any life-threatening injuries. Things we worry about will be internal bleeding, lung injuries, blood loss and things of that nature. Once the patient is stabilized in what we call the ‘golden hour,’ which is one of the most critical points to have the patient be stabilized, then I get involved. From a trauma standpoint most traumatic injuries can also be limb threatening, and some of them can be life-threatening if they have a worsening infection and things of that nature. But once the patient is stabilized by the trauma surgeons, my charge is to stabilize the limb, save the limb and get patients back to their normal baseline in terms of functioning. Things we see will be open fractures, which will be considered an emergent need to go to surgery to wash out the wound, remove any contaminants and stabilize the bone. We have medical conditions like compartment syndrome where the pressure in the leg continues to build up and this can usually lead to necrosis of the muscle or what we call ‘muscle death,’ if not addressed right away. So as an orthopedic trauma surgeon, I have to be very versed in the diagnosis and treatment of all this emergent conditions. And also, there are times when at 2 or 3 a.m. in the morning, we have to go back to the hospital to stabilize an unstable open fracture. Again, the most important thing is to save life, preserve limb and return the patient to function and that's something that I really enjoy doing.
