Here is an interesting case from tom oetting at the university of iowa. We had plannd to place a toric single piece acrylic (SPA) IOL but noted that we had a radial tear during I/A. as such we aborted the toric IOL and placed a standard SPA IOL. I was concerned that we may extend the tear posterior while getting the toric into final position and did not feel that it was worth the risk. As such we placed a SPA in the bag and moved the haptics such that they were not in the area of the radial tear where the bag is less clearly defined. I think we had 3 options here: 1. we could have simply placed the toric IOL and rotated it into position. most likely wold have gone alright. 2. we could have placed a SPA in the bag as we did 3. we could have plced a 3 piece IOL in the sulcus. What wold you have done? Thanks for any comments. tom o
