Clinical cases are used to illustrate multilevel dysfunction, microbiome shifts, diagnostics and clinical staging of chronic gastrointestinal conditions in dogs. The need for multimodal management is also addressed. Chronic enteropathy (CE) is the most common cause of recurrent or persistent gastrointestinal (GI) signs in dogs and cats. The condition is characterized by multilevel dysfunction that includes decreased mucus production, epithelial cell injury with loss of intestinal wall integrity, dysbiosis, and other overlapping pathologies. Histopathologic changes seen in intestinal biopsies from dogs with chronic GI signs overlap across CE categories and do not predict treatment response. The fecal Dysbiosis Index, however, is a practical tool veterinarians can use to assess shifts in the intestinal microbiota and changes in the intestinal environment. Dysbiosis is common, but not always present, in CE cases. Using two case studies from his internal medicine referral practice, Dr. Procoli shares his experiences in diagnosing and managing veterinary patients with nonresponsive chronic enteropathy.
