Follicular neoplasm of the thyroid is a term which is used by the pathologist when they cannot say whether it is cancer or not. There are lot of situations where it is difficult to tell for the pathologists based on a needle biopsy because the tissue available is very small and there are certain features they cannot appreciate when they do just a needle biopsy. So whenever a pathologist has given you a report which says follicular neoplasm there is a need for surgery. The risk of cancer in these patients is about 20% and that 20% necessitates a surgery to prove whether it is there or not. So the surgery could be either an open surgery to remove half of the thyroid or a complete thyroid and the same is sent for testing. There are situations where we can do a test called frozen section though it is helpful in very small number of patients which will give us a confirmation during surgery itself whether it is cancer or not. But it is not useful in all the cases. The surgeons and the pathologists are very selective in choosing the cases where it is possible to do. Quite often we do a total thyroidectomy removing the entire thyroid gland sending it for complete testing which will tell us whether it is cancer or not. For the patient it only means that the patient has to take lifelong thyroid hormone tablet which does not have any side effects. So in all cases of follicular neoplasm surgery is indicated.
