This was a challenging case of a giant hydatid cyst of the liver with cystobiliary communication in a 78 years old male. There was an incidental finding of malrotation of the gut. The cyst was bulging through the GB fossa. Due to the large cavity, we could not identify the cystobiliary communication. Roux-en-Y cystojejunostomy was done after the evacuation of contents and partial de-roofing of the cyst wall. Cholecystectomy & appendicectomy was added to the procedure.
