A tip for incomplete cleft lip repairs: The nostril can look much larger on the cleft side as you begin your markings. DON’T use the other nostril as a template for deciding how much cleft nasal sill to remove before you’ve repaired the muscle! The columella will pull toward the cleft side, making the noncleft side larger/wider. Sometimes then, no sill needs to be excised. Perhaps this could be predicted, but I don’t think I could do so reliably. Thus I place the peak of my skin mark at the groove just below the sill. If at the end of the skin repair you have a little excess, or need to reorient the alar base at all, then and only then should you take any sill out for final refinements.
