This young male was involved in a road traffic accident and subsequently had pain in his right lower face with difficulty opening his mouth. He presented to Dr. S. M. Balaji, Maxillo-Craniofacial Surgeon at Balaji Dental and Craniofacial Hospital, Teynampet, Chennai where he underwent a 3-D axial CT scan, which revealed a vertical fracture of the ramus of the mandible on the right side. An open reduction and internal fixation of the fracture through an intraoral approach was planned. Under general anesthesia, intermaxillary fixation was done to hold the fracture segments in stabilized position. A right-sided mandibular vestibular incision was then made. A mucoperiosteal flap was elevated and the site of the vertical fracture was clearly visualized. The fracture was then repaired using plates and screws. The screws were tightened with an angulated screw driver. Hemostasis was achieved and the wound was sutured.
