A common question Dr. Daniel Shapiro receives from Asian patients is, 'How do I make the tip of my nose sharper or more projected?' Specifically, rhinoplasties are very delicate and very demanding procedure. When you're looking for a rhinoplasty surgeon, make sure that you find a surgeon that you like their before and after pictures online; that you see a lot of patient reviews that they're happy with their rhinoplasties; ask your surgeon frank questions such as “how many rhinoplasty surgeries do you do on average?” Dr. Shapiro performs many rhinoplasties and a certain proportion of them are Asian rhinoplasties. An Asian rhinoplasty is specifically a different situation because most commonly, Asians lack adequate dorsal projection and tip projection so their nose does not have enough projection off their face and the tip is often flat and very close to their face. In those situations, Dr. Shapiro will place a cartilage graft and that graft either comes from the nasal septum or from the ear. The first reason why he does so is to provide adequate tip projection, which means that he will put a cartilage graft in between the medial crura, therefore it pushes out the nasal tip to provide more projection. He will also take some of that cartilage from the septum or the ear and place that on top to give more projection and a sharper angle. Almost everyone would like to have more defined nose, more of what we call a model's nose, and that cartilage can be sutured into place and when done expertly, you can't tell that it’s been done. It provides a sharper, angular result, which oftentimes it’s what patients are seeking. Frequently, Dr. Shapiro places a dorsal graft, either cartilage or some kind of a silicone graft, along the dorsum and provides cartilage grafts along the tip to give some of that sharpness to the angle.
