Refractory Epilepsy - What Is The Optimal Treatment? I had mentioned that for seizures the treatment is symptomatic? Medicines, anti-epileptic medication. That is the main treatment. Second is to find the cause of it and the treatment for the same. So either because of a stroke or a brain tumor or if it is because of an infection - treatment for that has to be given. Apart from symptomatic treatment options for causative factors, for some people if we observe, they will get it continuously, they will get it a lot and they will take a lot of medication. Such a group of patients only are patients with 'refractory epilepsy'. That is the term we use. If we look at what refractory epilepsy is, some patients will take a lot of tablets. That is, many groups of medications. Even after taking two groups of medications in a month, if more than two times they get seizures, we call them patients with 'refractory epilepsy'. For them what type of treatment can be given? Should a third group of medications be added? Should a fourth group of medications be added? No matter how many medications we add, their seizures will not stop and will continue. The treatment will be totally different for them. For such patients with refractory epilepsy there is an option. It is surgery. Do not be shocked that surgery is suggested for seizures because for refractory epilepsy, the treatment option itself is surgical treatment. The reason I say this is because for those who are already taking two groups of medications be it third group, fourth group, how many ever group of medications are added, the medicines side effects will remain but they will continue to get seizures. Every time they get seizures, that will affect the brain. There will be damage in the brain. Sometimes, sudden death can also happen. For a person who gets seizures there will be a lot of activities for the muscles. So when the muscles are active, the heart can stop or the heart can beat faster. In such cases, there can be issues with cardiac output. We call it SUDEP, Sudden death during epilepsy. We should avoid seizures. That should be our primary goal. For those who get seizures even after taking a lot of medication we do special investigations to find out the reason behind it. If the reason is a scar or wrinkle in the brain, through surgery if we remove the scar or the cyst, we will be able to provide them with a good solution.
