RECURRENT CYSTITIS: Bladder Fulguration Treatment for Recurrent Urinary Tract Infections

Recurrent cystitis is defined as having urinary tract infection (UTI) at least 3 times a year or 2 times at last 6 months. Unfortunately most of our patients had the situation far more than this. They use antibiotics and lots of supplement several times without finding a permanent solution. We perform cystoscopy to these patients to understand the underlying cause of the disease. Most of the time we find 2 abnormalities in the bladder. One is leukoplakia (or squamous metaplasia) which is uncancerous tissue differentiation of bladder mucosal cells and it occurs due to chronic irritation of urinary bladder usually by bacteria. The patients usually have some tendency to get UTI. Many women have UTI in some part of their life but some of them can not get rid of it completely. If we find leukoplakia inside the bladder which is puffy white plaques located on the bladder trigone (entrance of the bladder) or trigonitis which is hyperemia and patch change of normal bladder mucosa we know that there are embedded bacterial colonies inside these tissues. Interestingly most of these patients have a previous cystoscopy history. The urologists do cystoscopy to check if there is any abnormal tissue causing UTI, but uptill now leukoplakia or trigonitis were not considered as the cause of recurrent UTI among in urology society. Recently some centers published scientific papers proving the benefit of the fulguration of bladder to clean abnormal tissues in the bladder. (see the pictures) In our clinic (Istanbul Urology Clinic) we are treating RUTI by endoscopic evaluation for last 15 years. We frequently find leukoplakia tissues or inflamated mucosa (trigonitis) We fulgurate these tissues by Plazmakinetic bipolar energy. We presented our findings in Annual Meeting of the Turkish Urological Association 2020 120 patients with RUTI were enrolled into the study 55 patients undergone endoscopic evaluation 40 patients had leukoplakia 15 patients had trigonitis The success rate of Bladder Electrofulguration was 80% overall 90% for Leukoplakia lesions 60% for trigonitis patients So we concluded that Endoscopic Electrofulguration of the bladder mucosa has significant success for the treatment of Recurrent Urinary Tract Infections compared to conventional antibiotic treatments.
