The Art and Science of Diagnosing Nasal CSF Leaks

While we have highly successful techniques to repair nasal cerebrospinal fluid (CSF) leaks, the process of diagnosing CSF leaks can be problematic if the appropriate steps are not followed. First, take an accurate and detailed history to develop your clinical suspicion for a nasal CSF leak. Next, CONFIRM the CSF leak, THEN LOCALIZE the leak site along the skull base (bone separating the brain and sinus cavities). This presentation was a lecture I gave to my residents recently. It provides an overview to the most well-established diagnostic modalities, and provides a simple and evidence-based paradigm for diagnosing nasal CSF leaks. One key point is that NO test is perfect. There is both science and art that goes into the decision-making. Hopefully this helps someone out there!
