Seminar Full Title: The UW Health Learning Health System (LHS) - Realizing the Promise of a Primary Hospital Provider Team for Complex Patients Implementation Science Seminar Series | University of Wisconsin - Madison | December 2025 The Implementation Science Seminar Series is an interactive, monthly seminar (via Zoom) to learn about the four pillars of the Dissemination and Implementation field: implementation theory, implementation methods, implementation strategies, and implementation laboratories. Seminar Description: This session provides an overview of what it means to bring an LHS approach to a health care improvement project and describes the Primary Hospital Provider Team project’s pragmatic trial design and mixed-methods approach to evaluation, highlighting how these strategies are being used to assess the effectiveness of a proposed process redesign at UW Health. Speakers: Anne Gravel Sullivan, PhD, serves as the Associate Director of Research Translation and Implementation for ICTR’s nationally-recognized Dissemination and Implementation Launchpad, as well as its new Learning Health Systems program. With a passion for curriculum and systems design for impact, Dr. Gravel Sullivan works to ensure these programs provide the next generation of biomedical and behavioral scientists with formal training and other services to advance their careers, accelerate their impact into translational research, and improve health for all. Dr. Gravel Sullivan’s career spans 25 years of experience creating, managing, and teaching in professional development programs, including through distance education. She has held leadership roles in medical education for 15 years, most recently as the Executive Director of Distance Learning and of the Review Committee for Anesthesiology at the Accreditation Council for Graduate Medical Education. Prior to that, she served as Graduate Medical Education (GME) Program Manager for the University of Wisconsin Hospital and Clinics, as Education Administrator with the Department of Medicine, and as an Assistant Researcher with the UW’s Center on Education and Work. She earned her MA in Communication Studies from Northern Illinois University and doctorate in Speech Communication from Pennsylvania State University. She also holds a certificate in Distance Education Professional Development from the University of Wisconsin. Sara Westergaard, MD, MPH, is a faculty member in the Division of Hospital Medicine within the Department of Medicine. Her professional interests focus on enhancing team-based models of care to improve patient outcomes and promote workforce well-being and retention. She is a member of the Hospital Medicine ReEngineering Network (HOMERuN), the Society of Hospital Medicine, and the Society for General Internal Medicine. Prior to joining UW, Dr. Westergaard practiced clinically in California and Maryland. She also volunteered with the Global Health Service Partnership—a collaborative effort between the US Peace Corps and Seed Global Health—where she taught medical students in Uganda and Tanzania for two years as part of a workforce capacity building program.
