At first glance — a routine stage of rectal surgery: para-aortic lymphadenectomy and division of the inferior mesenteric artery. Vessel skeletonization with an ultrasonic dissector, careful clipping — everything seems precise and well controlled. But it’s often at these stages that unexpected situations arise. 👉 In this case, while introducing a loaded clip applier through the trocar, the clip accidentally slipped out. The surgeon tried to reposition it manually, but during clipping the clip shifted slightly — resulting in vessel injury and bleeding. Fortunately, the situation was quickly managed: clips were placed proximally and distally to the defect, achieving hemostasis. But such a minor detail could have led to a major complication. 💡 Tip for young surgeons: If a clip slips out during insertion — don’t try to fix it yourself. Hand the clip applier back to the scrub nurse and ask for a reload. This saves time and prevents dangerous complications.
