Location: Chrcynno (Dropzone near Warsaw), Poland. Time: 14.06.2019 CAMP ORGANIZERS STATEMENT on the incident of stall and damage of airplane SP-WAW during dropping jumpers on 14 June in Chrcynno. Organisation On that day “head down skill camp” was taking place, with 14 participants exiting plane as one group. It was second such camp organized this year at the same dropzone. First one took place two weeks before (31 May 2019) and 10 jumpers were taking part in it. On both camps, before jumps commenced, briefing was conducted covering the following topics: how to prepare for such jumps, jump phase, procedures in the airplane, exit and formation building, break off and landing. Additionally, before each jump exercises took place on earth (“dirt dive”) containing simulation of exiting the plane, exit and formation building. Exit of formation in plane looked as follows: 5 people (floaters) outside the plane, base (4 people) in the door and 5 people (divers) in the front part of plane. Such exit composition was agreed and consulted with pilot and jumps organizer. During exercises on the ground it was underline that it is necessary to keep separation between divers (5 people) and base. Divers could start moving toward the doors in the moment of breaking floaters away of the plane. In such configuration 3 jumps were executed. In second jump pilot noticed big “swing” of plane during jumpers exit. To avoid that in next jumps, it was agreed that plane speed will be increased and separation between divers and base will be increased as well. Next jump (third one) was executed seamlessly. In fourth jump jumpers performing divers and floaters roles were replaced. The same as before, all was prepared and briefed on the ground, together with separation in the plane and exit. Jump During taking place in doors of base it was noticed that plane starts slightly to tilt to the left and it was realized something disquieting is happening. In this exact moment base was pushed out from the plane by LO, a moment earlier than planned. Right after that rest of jumpers being outside of the plane fell off the plane. After exiting plane it could have been noticed that plane is moving on the tail and then through back to vertical movement with nose directed to the ground. After landing of all jumpers it turned out that two of them have leg injuries. Right after that plane has landed safely at the Dropzone. After video recording analysis and jump participants and pilot conversation, the incident looked as follows: Divers group (5 people) did not keep agreed separation with relation to base in the plane. It might have influence on adverse weight arrangement and plane stall. One of jumpers exiting plane during its vertical movement to the ground, has damaged height elevator, probably blocking it For both jumpers immediate medical assistance was provided, ambulances were called and both were taken to hospital. It was diagnosed that injuries are not severe and both left hospital and went home on the same day. Conclusion During organizing big formations jumps, it is necessary to pay special attention during the briefing on potential result of not keeping the right separation and improper weight arrangement in the plane during jumpers dropping. PILOT’S STATEMENT On 14 June 2019 at 14.34 pilot was executing 6th flight on that day, with 15 jumpers on board. After getting approval for a drop, pilot has turned on “1 minute to drop” signal and started configuring plane to a drop. In agreed drop point, he has turned on signal “execute jump”. During moving of jumpers toward exit, he was trimming the plane with simultaneously yoke push to avoid plane stall. Because of too big amount of jumpers (14 people) in back part of the plane, full yoke push was not enough to counteract stall. Speed was decreasing rapidly and plane has stalled into spin. Retrieving normal horizontal flight was not possible because of blocking elevator. Plane has started to dive rapidly and retrieving was not possible despite full yoke pull and full trimming (nose up). During diving VNE speed has been exceeded. At altitude of ca 1 km, after using big strength to pull yoke, height elevator was unblocked and plane has initiated horizontal flight. After making sure of plane steering ability, pilot decided to plane at the airport in Chrcynno. Landing was successful with one jumper on board. An inspection after flight has showed damage of horizontal stabilizer caused by collision with jumper breaking off the plane during stall. CONCLUSIONS Incident was caused by disobedience of safety rules – incorrect arrangement of jumpers weight before exiting the aircraft and disobedience of procedures despite conducted by pilot training before taking off.
