The images demonstrate two hemicords divided by a fibro-ossues septum with a small volume of fatty marrow at it’s posterior aspect. The two hemicords are symmetric in this case, and subsequently reunite inferior to the septum approaching the level of the conus tip. For each hemicord, there is both one ventral and one dorsal nerve root. This is an incidentally detected case of diastematomyelia in a 56-year-old female that presented for signs and symptoms of meningitis. Had the two hemicords demonstrated both two ventral and dorsal nerve roots, a diagnosis of diplomyelia would be appropriate.
