DBS or deep brain stimulation surgery in Parkinson’s disease is a very effective treatment. To usually resolves most of the symptoms of Parkinson’s disease, like tremors, bradykinesia and rigidity. It improves the motor fluctuations, reduces drug induced dyskinesia, minimises the hallucinations and the psychosis in the form of reduction in post op medicine requirement. Usually we target STN and subthalamic nucleus or GPA nucleus in Parkinson’s disease. We use high frequency stimulation either monopolar or bipolar using different contacts in STN nucleus to resolves the features of Parkinson’s disease. The advantage of DBS as compared to the older generation lesioning is that this is reversible. That means you can reverse the symptoms by adjusting the current you can programme it by external programing device by controlling it, you can control the frequency the contact we are using and the pulse width and also the type of current, either monopolar or biopolar or sometimes we can use advanced programming in the form of interleaving or MICC to adapt in the changes of Parkinson’s disease. In effect DBS is very useful in controlling the features of advanced Parkinson’s disease. It has been an important tool in therapeutic Parkinson’s disease, in these days. Nowadays we are having a new device called as directional current where we can steer the current in various directions of STN nucleus to minimise the side effects. We know for sure that DBS will also have some amount of side effects because the STN will also carry other networks of behaviour. By steering the current to specific directions in STN, we can minimise the side effects. So we have new technology coming in, in terms of giving better therapeutic solutions to Parkinson’s disease. We have a battery which can last upto 25 years. Then we don’t need to change the battery like how we used to do few years back. So that is an advantage these days in terms of deep brain stimulation surgery and in Parkinson’s disease.
