Pronator Syndrome (PS) Entrapment of a nerve can lead to a series of physiological issues, resulting in pathology, then anatomical changes and then further pathology. Pronator Syndrome (PS) Compression of median nerve as it passes between ulnar and humeral heads of pronator teres muscle or under proximal edge of flexor digitorum superficialis (FDS) arch. Relatively rare. Archers (repetitive flexion of bow), baseball pitchers, throwing athletes, racquet sports, weightlifters and bodybuilders. Sources include: Trauma, congenital abnormalities, or pronator hypertrophy. Also, ganglion cyst. Typically is not diagnosed for months or years. Symptoms Insidious Onset. Aching pain in proximal, anterior forearm. Paresthesia in fingers innervated by median nerve (Thumb, index, middle and lateral half of ring finger). Aggravated by repetitive pronation and supination. Pronator Syndrome (PS) Vs. Carpal Tunnel Syndrome PS - Loss of sensation over thenar eminence. (Palmar cutaneous branch arises proximal to transverse carpal ligament). PS - Negative Tinel's at wrist. PS - Typically negative Phalen’s. PS- No nocturnal symptoms. PS - Pain on resisted pronation from a neutral position, especially with elbow extension. PS - Direct pressure on pronator teres with forearm supinated may elicit paresthesia in median nerve distribution within one minute. PS - If resisted contraction of FDS to middle finger reproduces symptoms, median nerve compression at the level of the fibrous arch between heads of the FDS might be suspected. Pronator Teres Stretching: Elbow in Flexion Work through a symptom-free range of motion. Patient standing or seating in good posture. Elbow flexed to ninety degrees. Supinate forearm as far as possible. Use light pressure with opposite hand to slightly increase stretch. Hold a mild, comfortable stretch for ten to sixty seconds. Pronator Teres Stretching: Elbow in Extension Work through a pain-free range of motion. Patient standing or seating in good posture. Elbow extended. Supinate forearm as far as possible. Use mild pressure with opposite hand to slightly increase stretch. Hold a mild, comfortable stretch for ten to sixty seconds.
