Mini Y, Maxi Relief: When the Bronchus Needed a Pair of Shorts

A 39-year-old patient presented with complete stenosis of the left main bronchus, located just proximal to the interlobar carina. The patient had progressive exertional dyspnea and recurrent pulmonary infections over the preceding months. Bronchoscopic evaluation revealed a fibrotic, idiopathic stricture causing complete occlusion of the bronchial lumen. An initial attempt to restore airway patency using a straight silicone stent (13 × 40 mm) proved unsatisfactory due to poor adaptation to the complex bronchial anatomy. Subsequently, a tailored silicone Y-stent (size 14–10–10 mm/1 - 0,5 - 0,5 cm) was successfully deployed endoscopically to maintain ventilation to both the upper and lower lobes of the left lung. The procedure was well tolerated, with immediate clinical improvement. This case highlights the utility of customized silicone Y-stents in the management of complex benign bronchial stenosis involving the interlobar carina.
