A woman had upper and lower blepharoplasty, but is complaining about her wrinkled skin texture at the outer and inner edges of her eyes. She also has a piece of skin at the edges that pulls her eyelids. She wants to know if these can be fixed. Dr. Amiya Prasad, an oculofacial cosmetic surgeon, explains that eyelid surgery specifically does not address wrinkles - it addresses excess skin or in situations such as Asian eyelid surgery which involves the creation of a crease or fold in the upper eyelid. If someone has a lot of extra skin, there may be some improvement in the appearance of wrinkles but a lot of times, it's because of extra skin which had the wrinkles was removed. With the intention of improving wrinkles in the lower eyelids, a lot of plastic surgeons will remove skin. Unfortunately, with 20 years of experience, Dr. Prasad says that it is not the best choice. Many surgeons will remove skin which reduces the amount of skin that the lower eyelid requires for it to function well. The removal of skin affects the shape and contour of the eyes, which leaves patients left with lower eyelids that look like they are pulled down. This condition is called lower eye lift surgery or lateral scleral show. Lower eyelids need to be in a good position for them to function. Eyelids that are pulled down laterally in the outer corner diminish its effectiveness in protecting the eye and providing a platform for tear film distribution over the cornea. Patients with this type of condition may have dry eyes and other symptoms. Dr. Prasad suggests the use of tear drops regularly. He also recommends that she should communicate with the doctor who performed this surgery and see if any corrective procedures are necessary to address her problem.
