Ultrasound reveals internal particulate echoes, suggestive of debris, with posterior acoustic enhancement. Color and spectral Doppler confirm the absence of vascularity, ruling out vascular anomalies. The most likely diagnosis is a Baker’s cyst, a benign fluid collection arising from the gastrocnemius-semimembranosus bursa, often linked to joint conditions like osteoarthritis. Differential diagnoses include ganglion cyst, synovial cyst, adventitial cystic disease, hematoma, abscess, and lymphatic malformation. Prognosis is generally favorable, with conservative management or aspiration if symptomatic. Addressing underlying joint issues is key to preventing recurrence.
