When I first started to use the Visian ICL phakic lens implant in 2006, it was a different time with an older generation design. There were no fenestrations in the lens so we had to make two small peripheral iridotomies in order to prevent pupillary block. In addition there was no toric version available so for patients with large degrees of myopia and corneal astigmatism we had to do a bioptics approach: make the LASIK flap (but don't lift it), then implant the phakic IOL leaving the patient with mild myopia, then when healed, lift the LASIK flap and treat the remaining myopic cylinder correction with the excimer laser to address the corneal astigmatism. That is no longer necessary since we now have access to the toric version of the Evo Visian ICL. This video shows the loading of the IOL as well as the delivery and alignment. Back, 16 years ago, I posted a video of how I could use a special technique to place all four corners of the Visian ICL using just the injector. What is your technique?
