#patientsafety #MedTwitter #MedEd #cognitivebias #DunningKruger MY PERSONAL EXPERIENCE I am sharing my experience and radiology studies to increase awareness and support for patients who have experienced an injury to their phrenic nerve. It is an area of medicine that is underreported and under studied, leaving most patients helpless. I had a phrenic nerve injury several years ago, from a surgery. A hemi diaphragm paralyzed from a Phrenic Nerve injury atrophies from lack of use. This weakened diaphragm allows abdominal organs to push up into the thoracic cavity. This is called diaphragm eventration. For me, it causes further pressure and pain in my chest. While some patients may have no symptoms, others like myself can be quite debilitated which affects my quality of life. My diaphragm is in a permanent paradoxical position which means that it does not expand during inhalation or exhalation. I have permanent pressure on my lung and heart. Lung Function Tests have shown that my FEV1 was 96% several years ago shortly after the phrenic nerve injury. Now, it hovers between 62 and 66%. Phrenic nerve injury is understudied and under reported and it seems easy to dismiss a patient's symptoms. This may be related to cognitive bias. There is very little research done on rehabilitation of severed phrenic nerves and paralyzed diaphragms. I have listed two surgeons who are on the cutting edge of medicine in this field by transplanting a nerve from the leg to replace the damaged phrenic nerve. I am not their patient, and always tell people to do their own research and advocate for what is best for them.
