Primary pre descemetic DALK for acute hydrops Soosan Jacob with audio

This video shows my technique for treatment of acute hydrops with primary surgery instead of the traditional wait and watch approach/ air or gas injection/ other techniques that are currently employed. Here, I use a modified technique of pre-Descemetic DALK. This allows the cornea to heal without scarring unlike traditional approach which heals by scarring. It gives a one step solution while giving quicker visual rehabilitation. In addition, it simultaneously solves the pathology by closing the Descemet's break and allowing the edema to subside; improves corneal structure and transparency, improves the topography, corneal optics and visual acuity of the patient and provides anatomical correction of the ectasia and thinning. The risk of a penetrating keratoplasty is avoided unlike in traditional treatment where a central scar if forms, necessitates a penetrating keratoplasty. It also avoids the costs and co-morbidity associated with a second surgery.
