Speaker: Dr. Rathan Shetty Topic: Stomal Stenosis post total laryngectomy In this video, Dr. Rathan talks about: a. Understanding Stomal Stenosis: Definition and Impact: Stomal stenosis, a narrowing of the tracheostoma, impairs breathing, secretion clearance, and voice prosthesis use, potentially requiring long-term stenting or tracheostomy tubes, leading to discomfort, social stigma, and increased maintenance needs for laryngectomized patients. b. Etiological Factors Contributing to Stomal Stenosis: Radiation therapy, excessive scar tissue formation, wound dehiscence with secondary healing, infection at the stoma site, hypertrophic scarring (especially in certain populations), inadequate initial stoma construction, and tumor recurrence can all contribute to stomal narrowing. c. Surgical Techniques for Primary Stoma Construction: Various techniques aim to create a durable, D-shaped stoma with adequate size, including separate skin incisions, skin flaps, and avoiding cutting through the tracheal cartilage to minimize circumferential scar contraction. d. Management of Postoperative Stomal Stenosis: Treatment ranges from conservative measures like serial dilation with tracheostomy tubes or stoma buttons to surgical revision using advancement flaps or skin paddles to excise scar tissue and enlarge the stoma opening. e. Timing of Stomal Revision and Surgical Approaches: While the ideal timing for surgical revision is debated, the primary goal is to excise scar tissue and reconstruct the stoma with techniques that minimize tension and promote epithelialization, often utilizing local skin flaps. f. Prevention Strategies and Postoperative Care: Meticulous surgical technique during primary stoma creation, including oblique tracheal stump section, adequate skin excision, complete mucosal coverage, and early removal of stoma sutures, along with diligent postoperative care and patient education, are crucial for preventing stomal stenosis.
