Internal Jugular Vein (IJV) Ectasia is the abnormal dilation of the internal jugular vein, typically benign and often discovered incidentally during imaging. It can be congenital or acquired due to factors like increased venous pressure or trauma. On ultrasound, the ectatic IJV appears as a smooth, dilated structure with anechoic or hypoechoic content. Color Doppler imaging shows uniform blood flow, with potential aliasing in severe cases due to turbulence. Spectral Doppler usually reveals a phasic waveform, though severe ectasia may cause flattening. A diameter over 10-12 mm is considered ectasia, with severe cases exceeding 20 mm. An IJV diameter of 24 mm suggests significant ectasia, which, despite being well above normal, is generally benign if no other abnormalities are present.
