This is a 45 year old male who was complaining of flashes and floaters in the right eye for about 2 weeks. About 4 days prior to this operation, he lost his central vision of the right eye. Flashes and floaters can be the first sign of a retinal tear. A retinal tear can develop into a retinal detachment. It is likely that he developed a tear, and in the following 10 days, developed a retinal detachment. Loss of central vision is consistent with his macula becoming detached. This is a great video as the retinal tear is very obviously seen. Tears are not always easy to see, much less video. During part of the vitrectomy, I am also looking for additional retinal tears. It is not uncommon to find more than one retinal tear. The first part of the operation is the vitrectomy. I remove most of the vitreous and especially the vitreous near the retinal tear. Air is then used to replace all the intraocular fluid; including the fluid underneath the retina responsible for the detachment. With the fluid removed, the retina is technically reattached. At this point, all retinal tears should be treated with laser. Laser does not work on detached retina. The last step to this particular case is to replace the air with intraocular gas. I often use SF6 at about 15% concentration. This will not expand, but will also more slowly absorb, compared to air. If you have any questions, please feel free to ask here.
