Herbst sleep apnea appliance - instructions

Jeffrey I Goldberg DDS PLLC I’m going to go over how to take care of the herbst appliance that you got in our office. Before putting the appliance in your mouth at night you are going to want to be sure to brush and floss your teeth, making sure that they’re very clean, before wearing the appliance. Wear it throughout the night. When you take it out in the morning, you’re going to want to brush it – with the brush our office provided; it has two sides, a flat side and a pointy side. In our office here we like using the pointy side for cleaning the inside surface and the flat side for cleaning the outside surface. When you’re brushing your appliance you’re going to cold water and soap (dish soap or hand soap – whatever you have is fine) rinse it off with cool water. Get it in the container we provided and I recommend leaving the lid off during the day, letting the appliance air dry. A couple times a week we recommend soaking the appliance with Efferdent, Fixodent, or Polident, any kind of denture cleaner is fine. Put one of the tablets in this container fill it with room temperature or cold water, put the appliance in, and let it soak a minimum of 20 minutes. It’s okay if you leave the appliance in there longer or throughout the day, nothing bad will happen to it. When you are ready to take the appliance out of the solution rinse it off very well, rinse out the container, and let everything air dry. Couple things with the appliance; keep it out of anything hot; no hot water, no hot cars (if you go on a trip), no ovens, microwaves, dishwashers, clothes driers. Keep it out of reach of anything that could harm the appliance like a dog or small children. With this appliance you’ll notice that it’s two colors; blue rests on the top teeth, red goes on the bottom teeth. It won’t go in upside down but just as a quick reference that’s why we have the two colors. With the Herbst appliance you will receive a couple different tools along with it; there will be two of these keys with the triangle on the top and an allen wrench. This allen wrench is used to make sure the screws that hold the arm onto the appliance are kept tight. We recommend to our patients, about once a month to check the tightness of these screws to make sure that they haven’t loosened up. Sometimes just through wearing the appliance; putting it in, taking it out, moving it around, can kind of loosen them a little bit – so we just want to make sure that they’re snug in there. Be sure when you check these screws that you turn the key very gently because you can strip the inside of the screw pretty easily. (I’m just going to go and make sure all four are tight.) Another thing with these keys that we got, these are only used to advance the appliance. To advance it; what you’ll do is turn these little washers that sit on the arm (you can see both of them right there). The way you’ll know which direction to turn these washers to advance the appliance is by this little arrow. You can see this on is pointing down so on this we will start from the top turning the key downwards, the direction that the arrow is pointed. We are going to start with the washer that’s in the back – getting the key in the hole at the top, turning it 180 degrees to the bottom, then taking the key out. Now we’ll go to the washer in front – again starting from the top, turning 180 degrees the direction the arrow points, taking the key out. What we do for one side of the appliance we are going to want to do for the other, however, you’ll notice, on this side the arrow is pointing the opposite direction. So on this side, again, we’ll start from the washer that is in the back, turn it 180 degrees upwards now – same direction that this arrow’s pointing; and then we’ll do the washer that’s in front again making sure to turn it 180 degrees. Each time you turn both of those washers it will advance the lower jaw ¼ of a millimeter further forward. Dr. Goldberg and staff will direct you when to start advancing the appliance – so this is just to show you again what they would’ve gone over in that appointment.
