When patients get tests or imaging done suspecting scoliosis or other forms of spinal conditions, they are often found with a diagnosis that includes herniated discs. Often, these patients believe their only option is surgery. The disc's primary function is to facilitate the flexibility between the spine's vertebrae while still protecting what's inside the spinal cord, such as the nerves and other things that exit the spine that allows our bodies to function properly. The structure of the spine is designed to allow for movement without friction and degeneration and to act as a shock absorbent, which is one of the primary goals of spinal discs. So when the spine's tough inner gel pushes through the spine's soft inner layer and protrudes through the center of the spine, we call that a herniated disc. Disc herniation happens when there is abnormal weight in the discs caused by pressure in the vertebrae above or below it. It can also occur when discs lose hydration and are no longer able to maintain their normal position, causing the disc to herniate. This leads to nerve compression, irritation, and impingement, as the spinal nerves and canal are affected. Herniated discs, especially when there is compression, can create a great amount of pain, such as radiating pain or numbness that extends away from the spine into the extremities of the body. However, disc herniation doesn't always come with pain, which can cause the problem to worsen as the disc becomes weaker and goes undetected. Most surgeries to address herniated discs involve removing parts of the spine to decompress the nerve tissue. However, often the problem is not the disc but what the disc is pressing on. One of the most common surgeries is the discectomy. This is done by removing a portion of the affected disc or the entire disc to remove the pressure from the affected area. A laminectomy is often combined with a discectomy. This implies moving the posterior part of the vertebral body; this allows the nerve lamina to come out and provides more space, so the nerve is no longer compressed. Cervical corpectomy is another surgery where the adjacent vertebra to the affected disc is removed to relieve pressure. In this case, doctors remove a bigger area of the spine. A laminoplasty is a procedure to create more space in the spinal canal and reduce pressure this way. Lastly, we have the traditional spinal fusion, which implies fusing multiple bones to make one single bone using screws and plates. There are non-surgical treatment options available that include: Chiropractic care Therapy At home Exercises However, general exercises don't affect the alignment or hydration of the discs, and to deal with the cause of disc herniation, spinal alignment, and disc hydration should be the priority. An alternative treatment is cyclic therapy with traction, which helps rehydrate the discs with its motion. In disc herniation cases, it should be a priority to help the disc regain its normal shape. Then, realigning the spine will help remove the disc's uneven pressure, which helps it return to its normal position. However, this process doesn't happen overnight, and for patients with debilitating pain, sometimes the only answer is to have surgery at that point.
