The wrist hyperflexion and abduction of thumb (WHAT) test was developed by Jean Goubau (Goubau, J. F., Goubau, L. A. U. R. E. N. T., Van Tongel, A., Van Hoonacker, P., Kerckhove, D., & Berghs, B. (2014). The wrist hyperflexion and abduction of the thumb (WHAT) test: a more specific and sensitive test to diagnose de Quervain tenosynovitis than the Eichhoff’s Test. Journal of Hand Surgery (European Volume), 39(3), 286-292) as a test for de Quervains. He developed this test to have more focus on the abductor pollicis longus (APL) and extensor pollicis brevis (EPB) with an active movement. This is different from the Finkelstein test which is a passive test which stresses other joints in the area. Additionally, this test allows the patient to have control over the pain produced by the test, rather than the person performing the test, which is the opposite of the Eichhoff maneuver. How to perform the test -Support the patient’s elbow -Have the patient fully flex their wrist with the thumb extended and abducted -Apply a gradually increasing resistance force to the thumb A positive is a production of pain Goubau did compare the test to Eichhoff. The WHAT was better in terms of accuracy (94% compared to 84%), sensitivity (99% compared to 89%), and specificity (29% compared to 14%). It also has a lower false positive rate (71% compared to 86%) which the Eichhoff is said to have a high false positive rate. The drawback to these results are that it’s just one study and the testers were experienced hand surgeons. What do you think of this test? Do you use it?
