What is an Oral Biopsy? Oral Biopsy is a surgical procedure to obtain tissue from the patient’s oral cavity, for microscopic examination, usually to perform a diagnosis. Basically the reasons for performing a biopsy are i) To establish a definitive diagnosis as early as possible so that correct treatment may be initiated without delay ii) to establish a prognosis in malignant or premalignant lesions iii) To determine whether an abnormality has been completely removed iv) act as a document with medical legal value Indications for Biopsy 1. Any Lesion that persists for more than 2 weeks with no apparent etiologic basis 2.Any inflammatory lesion that does not respond to the treatment even after 2 weeks 3. persistent hyperkeratotic lesion( A bump or patch of thickened skin is known as a hyperkeratotic lesion). 4. Any lesion suspected as neoplasm. A neoplasm is an abnormal growth of cells known as tumor 5. Lesions of unclear aetiology, particularly when associated with pain 6. Lesions that interfere with oral function, such as fibrous hyperplasias and osseous lumps. 7. Radiolucent or radio-opaque osseous lesions. Types of Oral Biopsy There are six main types of Oral Biopsy Cytology: This type of Oral Biopsy can be used to diagnose lesions in the oral cavity due to post-radiation changes, herpes, and fungal infections. Cytology allows examination of individual cells but cannot necessarily result in an accurate and definitive diagnosis. It is recommended that this type of Oral Biopsy be performed along with an Excisional or Incisional Biopsy. Aspiration Biopsy: In this type of Oral Biopsy, the oral surgeon uses a needle and syringe to remove a sample of cells or contents of a lesion. If the oral surgeon is not able to withdraw fluid or air it probably means that the lesion is solid. Incisional Biopsy: If it is larger than 2cms, an incisional biopsy is advised which includes representative areas of the lesion along with a healthy normal margins (2/3rd lesion and 1/3rd normal tissue). When a malignant lesion is suspected, incisional biopsy technique is mandatory Excisional Biopsy: This type of Oral Biopsy is performed for small oral lesions, usually less than 1 cm. On clinical exam, the lesion appears benign. This type of Oral Biopsy results in complete removal of the lesion. Punch Biopsy: Punch biopsies are commonly used in dermatology for sampling of skin lesions. It is also used for gingival biopsies especially for cases of pemphigus vulagaris or mucous membrane pemphigoid. Brush Biopsy: It is a non invasive procedure. In this type of Oral Biopsy, firm pressure with a circular brush is applied, and rotated give to ten times, causing light abrasion. The cellular material picked up by the brush is transferred to a glass slide, preserved and dried. Contra-indications for biopsy 1. When the general condition of the patient is very poor. 2. When acute, virulent, pyogenic infection is present 3. Pigmented lesion (melanoma) should not be biopsied as it may transform into malignant by cutting 4.
