INTERMAXILLARY FIXATION/ MAXILLOMANDIBULAR FIXATION Facial fractures are common due to road accidents, trauma etc. Inter-maxillary fixation or IMF is an important in the treatment of maxillofacial fractures, and is usually applied by wiring together the fixed upper and lower arch bars or Ivy eyelet. IMF can be done by: 1. Direct interdental wiring 2. Ivy loop wiring technique / ivy eyelet wiring technique 3. Arch bar technique 4. Cap splinting in children 5. Gunning splint in edentulous 6. Intermaxillary fixation screws. In this video, we'll demonstrate ivy eyelet wiring technique There are 2 terms: Reduction [bring back to original position] and immobilization. In this technique, first we get teeth in occlusion or centric relation called closed reduction and then fixing it by wires/arches Immobilization Important points: •Intact dentition or jaw is used to assist in the reduction or realignment of the opposing dentition and arch. •In this technique , we get help from 2 teeth. •Teeth should be firm. •Clinically we pass from interdental spaces. •Always move clockwise for tying and Anti-clock wise for untying/opening. Total sites: 10 in whole mouth i.e; 5 in maxilla; 5 in mandible -5 sites in maxilla= b/w 16 and 17 , 14 and 15, 11 and 21, 24 and 25, 26 and 27 -5 in mandible =btw 36 and 37, 34 and 35, 31 and 41, 43 and 45, 46 and 47 Indications: -Reduce , allign and fix fractures -Help in immobilization of fractures -Guide for seeing occlusion Limitations: -Prolonged immobilization has been associated with trismus -TMJ ankylosis after using for prolonged time -Difficult oral hygiene maintainance -Unable or unwilling patient to tolerate intermaxillary fixation for any period of time Risk of aspiration in patient prone to nausea or vomiting Mostly healing time is 4- 6 weeks. #oralsurgery #lecturedental #wellness #brokenjaw #imf #intermaxillaryfixation #maxillofacialsurgery #intermaxillaryfixation #eyeletcurtains #eyelet #eyeletwiringtechnique #midfacefracture #jawfractures #BDS #oralsurgery #mandibularfracturemanagement #lecture #trauma #maxillofacialfracture #techniqueeyeletfabrication #dentallectures #neetmds #careafterimf #oralsurgeon #maxillofacialsurgeon #dentalsurgeon #health
