It's a technology which has evolved in the last two to three decades. We do this surgery for any person suffering from chronic rhino sinusitis causing lot of blockages in the nose and in the sinus ostium. The purpose of this operation is to relieve the blockage in the paranasal sinuses and to allow ventilation. We remove the diseases with the help of special equip powered instrumentation using the endoscope and with very good precision we operate removing only the disease part and minimally removing the normal healthy mucosa. It is called functional because we preserve the normal healthy mucosa and remove only the blocked obstructions and the narrowed ostium of the sinuses. Coming to the complications of this procedure it is a procedure which requires lot of skill and training and very good equipment and monitor and camera system so one should venture into this type of operations only after sufficient training because if you do not have a thorough anatomical knowledge or if the disease complicates the anatomy and distorts anatomy you can end up with complications. The purpose of this operation is for chronic sinusitis, nasal polyps, deviated nasal septum, any other mass or obstructions closing the sinusitis giving rise to severe headache. The commonest complication of the FESS operation is hemorrhage because nasal mucosa is very rich in vascularity and in the area where we will be operating it is close to multiple blood vessels and inadvertent injury can lead to severe hemorrhage or if the patient has a high blood pressure and he is not prepared properly then also he can end up with a lot of hemorrhage. The second most common complication is very quite dreadful injury to the vessel called anterior ethmoidal artery which runs in a mesentery and this runs into the orbit and if the injury to the medial rectus muscle happens the simultaneously the injury to this vessel happens and the vessel gets retracted into the orbit and the person can become blind. There can be hemorrhage into the orbit and if it is not recognized early and evacuated a person can lose vision. This is one of the most dreaded complication. Another complication being a CSF rhinorrhea because we are operating very close to the skull base. Injury to the cribriform plate or any other lateral lamella of the cribriform plate or ethmoidphobia can result in injury and a leak of the brain fluid called the CSF.
