Carpal Tunnel Syndrome Anatomy (Animation) | USMLE Step 1 | Median Nerve Compression Explained In this high-yield lecture, we explore the anatomy of the carpal tunnel and the pathophysiology of carpal tunnel syndrome (CTS), a common entrapment neuropathy frequently tested in USMLE Step 1. Understanding the detailed anatomy of the wrist and median nerve is essential for exam success and clinical practice in Western healthcare systems. We begin with the boundaries of the carpal tunnel: Floor and walls: carpal bones. Roof: transverse carpal ligament (flexor retinaculum). The structures passing through the tunnel are reviewed—median nerve, flexor digitorum superficialis tendons, flexor digitorum profundus tendons, and flexor pollicis longus tendon. Carpal tunnel syndrome occurs due to compression of the median nerve within this confined space. Clinical features include numbness, tingling, and pain in the lateral 3½ fingers, weakness of thenar muscles, and difficulty with fine motor tasks. We also explain diagnostic maneuvers such as Tinel’s sign and Phalen’s test. Management strategies range from wrist splinting, corticosteroid injections, and activity modification to surgical decompression (carpal tunnel release) in refractory cases. Clinical correlations include risk factors such as pregnancy, diabetes, hypothyroidism, rheumatoid arthritis, and repetitive wrist use.
