From the very start, Patches is in Murkwater Cave, which sits in the ravine north of Agheel Lake and just east of Murkwater Coast. If you want a clean, “no detours” route, the simplest reference point is Gatefront Ruins. From there, head north toward the river cutting through the landscape. You’re aiming for the narrow canyon with shallow water that runs under the main road. Follow the water upstream into the ravine until you see a small cave opening in the rock wall. That entrance is Murkwater Cave. Go inside and keep moving forward. You’ll hit a boss fog and the fight with Patches starts. The questline only “starts” properly if you do the thing Elden Ring never teaches you to do: stop attacking when he begs. Get him low enough and he’ll surrender, then you stop, let the surrender dialogue play out, and the fight ends. Rest at the Site of Grace afterward and he becomes a merchant in the cave, which is the whole point of sparing him. If you kill him, the “questline” becomes a short story about consequences.
