Date: December 4, 2014 Description: All too often people leave one care setting and do not successfully make it to another. To keep people from falling through the cracks as they transition to and from settings such as emergency departments, Indian Health Services, primary care, school counselors, outpatient mental health, and campus counseling centers, optimal care requires that there are standardized ways of helping the client, their caregivers, and the systems themselves overcome barriers to that transition. These include strategies like making a way to get an urgent MH appointment, getting informal caregivers to help in the transition, and more.
