Key messages: How do we avoid Perforations? •You need to always assess the degree of axial inclination of the tooth prior to access. •Probe around the tooth to determine the shape and orientation of the CEJ. •Be cautious in patients with limited opening which can result in unintentional gouging. From the tip of a #4 or #2 surgical round bur to where the tapered portion of the shaft meets the cylindrical portion is always 6.5 mm, making it a great depth gauge for locating the pulp chamber.
