In elderly, asymptomatic females, multiple calcific foci in the uterine myometrium seen on ultrasound are often benign and associated with dystrophic calcifications. These typically result from calcified fibroids (leiomyomas) due to age-related changes, particularly postmenopause. Uterine fibroids that undergo degeneration and involution frequently calcify as part of this aging process, making these findings incidental. Differential diagnoses include calcified fibroids (most likely), adenomyosis with calcifications, endometrial calcifications from chronic inflammation, and rare uterine sarcomas. Most likely diagnosis is calcified fibroids, a benign condition that usually requires no intervention. Management involves no treatment, unless symptoms arise.
