I do about 3-4 intraocular lens exchanges (IOLX) per week. Usually IOLX is pretty straight-forward for most IOLs. The LAL is just built different. It is unusually difficult to remove for a few reasons: 1 The lens periphery is thin and can be easily grasped with forceps for removal. The problem is that the thin edges crumble and break-off when we apply force to the LAL optic edges. 2 Once the lens edges crumble, then the mid-periphery of the lens is thicker making it difficult for most forceps to grasp the lens optic for removal 3 The lens haptics easily detach from the lens optic eliminating the option of pulling on the haptics to facilitate removal 4 The lens is slippery to the forceps and does not cut predictably like other acrylic or silicone IOLs So normally it takes about 2-3 minutes for me to remove or expant a lens implant. To explant this patient’s LAL, it took me 27 minutes.
