This song is about cotrimoxazole (sometimes called Septrin) is a combination of two antibacterial agents namely sulfamethoxazole and trimethoprim. The antimicrobial activity of the combination of these two antibacterial agents results from actions on sequential steps of the enzymatic pathway for the synthesis of tetrahydrofolic acid. Sulfamethoxazole is a member of a class of antibacterials called 'Sulfa Drugs' which are also known as the Sulfonamides. The sulfonamides are structural analogues of para amino benzoic acid (PABA) and competes with PABA for the enzyme dihydropteroate synthetase. This bacterial enzyme responsible for the incorporation of PABA into dihydropteroic acid, the immediate precursor of folic acid. Trimethoprim is a folate antagonist. Structurally it resembles the pteridine moiety of folate closely enough so that it acts as a substrate to the bacterial enzyme dihydrofolate reductase, which is many times more sensitive to trimethoprim than is the equivalent enzyme in humans. Dihydrofolate reductase reduces dihydrofolate to tetrahydrofolate, which is required for one-carbon transfer reactions. Coadministration of sulfamethoxazole (a sulfonamide) and trimethoprim in the form of cotrimoxazole introduces sequential blocks in the biosynthetic pathway for tetrahydrofolate; the combination is much more effective than either agent alone. Sensitive microorganisms are those that must synthesize their own folic acid; those that can use preformed folate are not affected. Cotrimoxazole is a broad spectrum antibiotic. It can be used to treat infections including a type of pneumonia called PCP which is caused by a fungus that affects the lung, infections associated with other lung conditions such as COPD and cystic fibrosis. It also reduces the risk of toxoplasmosis, an infection that can affect the brain.
