The 2025 European Society for Vascular Surgery guidelines outline management. Key is rapid access to 24/7 multidisciplinary teams competent in both open and endovascular techniques, operating in hybrid rooms. Initial care includes damage control resuscitation and early Tranexamic Acid for severe bleeding. Tourniquets are Class 1 for uncontrolled limb bleeding. REBOA is Class 3B, not routinely recommended for severe torso haemorrhage. For stable patients, CT Angiography (CTA) is the Class 1 first-line diagnostic scan, prioritised over delaying for potential kidney issues. ABI is not reliable in trauma. The main goal is Class 1 revascularisation to restore blood flow, preferably with primary definitive repair. Temporary vascular shunts are Class 1 for unstable patients to minimise time without flow. Repair uses open surgery or endovascular methods like stents or embolisation, chosen based on injury specifics and location (neck, chest, abdomen, extremities). Pediatric trauma needs specialised consideration. Recovery involves follow-up (often Duplex Ultrasound) and managing complex challenges. Significant evidence gaps remain for many areas.
