Sometimes one notices a tear in the roof of the tunnel in a clear corneal incision, which extends radially, often jeopardising the integrity of the water tightness of the clear corneal incision. The primary reason for this to occur is a thin superior lip. If this radial tear is self limiting or does not extend beyond a certain point and at least 1mm of the tunnel roof is intact, the clear corneal valve will hold and be effective, and one can get by without sutures and just stromal hydration. This case also shows a OCCI or opposing clear corneal incision in a patient with more than 1.0 D of astigmatism. After taking a clear corneal incision in the steep meridian another incision is made in the same meridian opposite to the main incision in order to take advantage of the additional flattening effect of this incision to combat astigmatism. This however is more easily said than done as the second incision needs to be performed with the non dominant hand
