Lifting a Hanging Columella and Refining the Lower Nose | Rhinoplasty I'm Dr Joseph Shvidler and I'm a double board certified facial plastic surgeon currently practicing at the Seattle facial Plastic Surgery Center in Seattle Washington when we lift to Hanging columella it involves bringing up the central portion of the nasal tissues between the nostrils that this is the definition of columellum it's usually most noticeable on the side view you see the nostril show on a side view and that could be due to several factors if this could be due to the fact that the septum is too long or overgrown or the lower airlock cartilages are misshapen and they're too long or too broad there's another situation where you can have retracted Ayla which is this portion of those and that may give you an illusion that you have hanging polyamala so sometimes you have to address the columella by reducing it and reducing the cartilages and sort of bringing the columella further inside your nose sometimes you actually have to add cartilage to the Ayla to bring the Ayla down in addition to bringing kalimela up to restore the alar polymeric proportion ideally you only want to see a couple of millimeters of your columella anything more than that does not look right patients are able to see these changes immediately after surgery the columella is not covered by a cast Maybe by tape but they should be able to see those results right away I think they really see the difference in photographs when they have side shots they're not as self-conscious when somebody's looking from the side or three-quarter view of their phase that's when it really starts to be noticeable https://www.westcoastface.com/nose/rh...
