Laparoscopic surgery also known as “Laser surgery” in the common parlance is a minimally invasive method of performing most abdominal operations. It involves the use of specialised equipment and instrumentation and makes the surgery safer and the recovery faster for the patient. The procedure in conducted under general anaesthesia where the patient remains unconscious and the muscles are relaxed. The abdomen is insufflated with a carbon dioxide gas to improve visualisation and also to create space for working inside the cavity. Several small incisions ranging from 5 mm to 12 mm are made on the abdominal wall for placement of ports which are devices through which the laparoscopic instruments are placed. It involves the use of a telescope which is inserted inside the abdominal cavity for visualisation of the intra abdominal organs. This telescope transfers the image to a camera head and then to a processor which digitises this image so that it can be visualised under magnification on a monitor by the operating team. The working instruments are long and their tips mimic the open surgery instruments which are controlled at the instrument handle. Numerous sophisticated energy sources are also used during the surgery for hemostasis and specialised devices like staplers are used to cut and seal tissues and to rearrange the anatomy. Since the visualisation is magnified and much better as compared to open surgery, the surgery can be done in a precise and meticulous manner. The surgery facilitates inspection of all the abdominal organs and sometimes concomitant procedures can be done at a single sitting through the same incisions. The ease of hemostasis and small incisions make way for lesser blood loss, less post operative pain and a faster recovery. Though the equipment and running cost of the laparoscopic procedure is more as compared to the open approach, the shorter hospitalisation, fewer complications and faster recovery make it cost effective in the long run. Moreover, since the entire operative procedure is visualised on a television screen, it facilitates recording the procedure and it can be transmitted for teaching and demonstration. Nowadays, most intra abdominal operations can be done through the laparoscopic approach. It can be used for general surgical procedures like gallbladder stones, hernia and appendicitis, gynaecological surgeries, oncosurgery, urology and even in complex gastrointestinal reconstructions and obesity surgery. It has a role in diagnosis, assessment and treatment of infertility and staging of intra abdominal malignancy. These days the laparoscopic surgery is also evolving and the era of reduced port and single incision surgery has begun. Here, the number of ports is reduced as compared to the conventional technique and in some cases, particularly where the cosmesis is an issue, it can be done through a single small incision usually concealed in the navel. Robotic surgery is another stage of evolution of laparoscopy where the surgeon is seated on a console away from the patient from where he controls the robotic arms at the patient side. This further has the advantage of better ergonomics, 3 D visualisation and greater dexterity of movement of the instruments which further improves the precision and the outcome of surgery.
