The same water that flows through the entire radiator system also flows through one side of the plate heat exchanger. Over time the metals in the system break down due to corrosion and cause sludge/debris in the system. Sometimes this sludge can build up and block up the plate. So now when the central heating water is trying to flow through the plate to heat it up, the water can not flow through properly, causing the boiler to overheat and turn off. The boiler then cools down and then tries to flow through again. This is what causes the hot water to come and go. A way you could tell if this was the cause is if you put your hand on the plate. If when the hot water is running, one side of the plate is hot and the other side is cold then this is a sign that it is blocked. Another sign that it is blocked is your radiators getting hot when you are running your hot water. In this instance what is happening is the hot heating water that should be flowing through the plate can't flow through, so instead it forces itself through the diverter valve and down into your radiators. From here you have two options, you can either clean it out or you can replace it depending on how bad it's blocked. 2.) It's pin-holed. Usually the your central heating water and your drinking water will be on two separate circuits inside the plate but on the very rare occasion the plate can pinhole which causes the two to mix. The main sign of this would be your boiler pressure keeps increasing too high (this is because the cold water is forcing itself into the heating circuit through the hole). If this is the fault then you would need to change the plate.
