Because the True Zenith world (all special and secret seeds enabled at the same time except for Skyblock) has its Underworld almost completely filled with lava, you can use that to your advantage by getting a Superheated Blood item (Lava Shark mount) to quickly move in it. I've blown up structures and terrain in the lava ocean to have enough room to comfortably move during this (and, previously, the Wall of Flesh) fight, both horizontally and vertically. My "arena" extends up to the middle main island of the Underworld and I use the chasm on the side of it to safely go around Mechdusa and go back to the other side of the arena, where I've also blown it up to make another chasm where I can do that again. On top of it, I added 3 heart statues for extra healing. Because Legendary mode usually doesn't buff enemies' ranged attacks, the boss' lasers barely deal any damage so it's not something you really have to pay attention if you have ~100 defense. However, touching any part of it deals over 100 damage even with 100 defense so avoid contact damage at all costs. Prime Vice is particularly dangerous if Mechdusa is on your left so once it speeds up to catch up to you just go at full speed and in a big zig-zag pattern to prevent it from touching and ripping your dick off. Besides that, the rest is more manageable. The Twin's attacks are slow when in lava or don't have that much range, their dashes are easily outpaced by the Lava Shark, the other parts of Skeletron Prime don't actively try to reach you as the Prime Vice, and their projectile, much like The Destroyer's body and proves, tend to miss and, again, are weak with a lot of defense. Skeletron Prime's head slows down when it's close to reach you so the only way it can touch you is if an obstacle stops you right before that happens. Otherwise it's not a threat at all. My armor is the Frost Armor and, for attacking, I'm using an Unreal Dart Pistol with Crystal Bullets. You can see my accessories at 09:30. I forgot to equip the Brain of Confusion so yeah, don't be like me in that regard.
